Application Forms
On this page, you can download some of the forms for the good cause exception application package. Applicants may use these forms in case they need duplicates or additional copies.
These forms should not be used unless the applicant has submitted a request for a good cause exception. The forms should only be used for individuals who have already received the application package. If you attempt to submit one of these forms without having requested a good cause exception, the forms may be rejected. Furthermore, you may miss the 30-day time frame for submitting a good cause exception request.

- Application instructions (.pdf). Complete instructions, including tips on how to avoid common mistakes.
- Main application form (.pdf). Please note the restrictions described above on downloading the application form. This form has a watermark to prevent inappropriate use, and it is provided here only for reference. If you need a copy of a good cause exception application form, please visit our Contact Information page and contact the Board staff.
- Reference forms (.pdf). Although the Board requires two reference forms, some applicants may want to submit additional reference forms.
